That's a good question Blubriz. I have been wondering the same thing. I suffer from Gourdis maximus (large head) and have a heck of a time finding hats that fit. No place seems to have measurements that correspond with the sizes.

There are
hat size charts available that can give you a rough idea.
But I've found that sizing varies from maker to maker, and even betweens styles by the same maker, so it's a crap shoot a lot of the time.
My head is sort of between standard sizes anyways, so maybe Im just harder to fit than most.

I have found that all makers have different sizing as well SlyBear. I would not be such a problem except I have a huge head. (Ghordus Maximus)

I like wearing fedoras, but I have had to retire a number of wool fedoras because wool causes my skin to itch.
Does anyone else have that problem?
Found any solution (other than wearing material that's not wool)?

hat sizes within a single company tend to run true, but between companies... not true.
one sie fits all ...doesn 't
adjustable hats, don't get big or small enough if your size is in the edges of large or small.
and XL orXXL does not really mean much. Just have to try them on.

Brownimation, I like fedoras, but don't have one. I imagine a wool hat would bother my skin too. Do they sell them with liners in them? It would seem like there may be a high-end version with a silk liner for comfort... but I haven't looked into it.

I believe you're right. A lot of high-end hats do have liners. I own one particular wool fedora that I really like but can't wear anymore because it does not have a liner.
I have played around with the idea of constructing my own liner for it, but have not attempted it yet.

my favorite cap is a gray wool knitted tight-on-the-noggin type of deal. my head is very small and hard to size for normally, so the homemade custom route is best for me. regularly wearing a hooded sweatshirt in cool or cold weather seals the deal.

I have a hard time buying hats online unless it's a brand/style that I've been able to test fit locally. But I'm a recent convert to Tilley hats and will likely be adding a couple more to my collection.
