I need some advice... and a kick in the butt!

Michael:
I used to work out and was in uber-shape (black belt in isshin-ryu) .  My family tens to run to the large, but I always managed to keep the weight and health in check.  but over the years laziness has put on the pounds.  Now diabetic and hypertensive, I am a mess. Any suggestions for diet, workout etc?  I work as a teacher all day, get home, help with my kids homework (one has special needs) that takes me to about 5:30, then the wife comes home and needs "time" to vent, etc.  And frankly, I am worn out by 6:00.  Any thoughts and suggestions would be greatly appreciated.  The wife is now a vegan (due to her Multiple Sclerosis) and that seems to be working grand for her.  Maybe I should try it?  I really need to exercise.  I have a nice Nordic Track treadmill that is being used as a clothes hanger, an assortment of dumbbells 15-35, and that's it.  Please advise.  Cheese, do you have any wise words?

More cardio workouts will help. My trainer has me working on kettlebells. I find my energy level (after 5pm) increasing.

What amount of weight?  Where do you get them?
