I'd get my dress shirts at the 2nd hand store.  Only 25 cents each.  They never have the right size either though, that's why I don't own a dress shirt.  I have been looking for a nice suit though.  They're only a buck.

I like Jos. A. Banks shirts - the 100 % cotton are a little more expensive but really wear well.
Land's End has some good ones too

Whats a 48R? Is there a Canadian equivalent or am I just an idiot.

measured around the chest right guys? 48r = 48 regular
